A new therapeutic vaccine has shown early promise in extending the lives of patients with aggressive brain tumors, with two-thirds of participants in a small German study still alive eight years on from their enrollment. Results from the long-term follow-up, published in the journal Nature, also showed that 42% of participants had seen no recurrence during that period, a figure that surprised even the researchers involved.
The vaccine was developed by scientists at the German Cancer Research Center, University Medical Center Mannheim and Heidelberg University Hospital, among other institutions. Its target is a mutation in the IDH1 enzyme that generates an abnormal protein structure known as a neoepitope. What makes that structure valuable as a treatment target is that it simultaneously fuels tumor growth and registers as foreign to the patient’s immune system, making it identifiable for attack.
Rather than preventing cancer from forming in the first place, the vaccine works by directing the immune system against tumor cells that are already present. Two arms of the immune response were engaged: T cells moved against abnormal cells directly, and antibody-producing B cells mounted a separate attack on the tumor. The aim, lead author Michael Platten explained, was to keep the tumor from returning once the initial course of treatment had been completed.
Standard care for brain tumor patients combines surgery with chemotherapy and radiation, yet even that combination offers limited protection against the most aggressive forms of the disease. Patients with high-grade tumors rarely reach the five-year mark after their diagnosis. All 33 participants carried grade three or four astrocytomas, a category of brain tumor that ranks among the most common cancers of the central nervous system and responds poorly to existing treatments.
Ulrich Herrlinger, director of neuro-oncology at University Hospital Bonn and independent of the research, described the findings as holding real promise for patients. He noted that high-grade astrocytomas return in virtually every case, progressing until treatment can no longer reach them, and that sustaining immune activity against the disease indefinitely could offer a credible path to holding it in check.
Regardless, both Herrlinger and Platten were careful to frame that optimism narrowly. Drawing firm conclusions from a group of 33 patients is not possible, Herrlinger said, and a larger controlled study with a comparison group will be necessary before the vaccine’s true effectiveness can be determined.
That study is already being planned, with a randomized trial scheduled to open enrollment in March 2027 and expected to draw in more than 200 patients. Definitive data is not expected to emerge for around nine years, Platten said, and whether repeat doses could deepen the body’s response to the tumor over time is a question that will only be settled then.
As the study of how best to use vaccines against hard-to-treat brain cancers continues, other teams at firms like CNS Pharmaceuticals Inc. (NASDAQ: CNSP) are also looking to develop more effective medicines that can move the dial in the way brain tumors are treated.
