Study Finds That Vitamin D Supercharges Chemotherapy Against Breast Cancer

A clinical trial in Brazil has found that adding a daily vitamin D supplement to standard chemotherapy improved outcomes for women with breast cancer. The finding contributes to growing interest in low-cost nutritional approaches that could enhance cancer treatment. Unlike many pharmaceutical agents designed to boost chemotherapy response, such an intervention would be widely accessible and inexpensive. The implications extend to any clinical setting seeking affordable ways to improve treatment response in breast cancer. 

Conducted at the oncology clinic of FMB-UNESP, São Paulo State University’s medical school, the trial enrolled 80 women over the age of 45. Participants were split into equal cohorts, one receiving 2,000 international units of vitamin D daily and one receiving a placebo. 

All participants underwent neoadjuvant chemotherapy, a protocol in which treatment precedes surgery to reduce tumor size. The trial was funded by FAPESP, and the findings were published in the journal Nutrition and Cancer

After six months, the gap between the two groups was striking. Among women who received vitamin D, 43% achieved complete disappearance of their cancer following chemotherapy. In the placebo group, that figure was 24%. Study author Eduardo Carvalho-Pessoa noted that the dose of 2,000 international units daily was substantially lower than clinical deficiency-correction protocols. 

That the effect appeared at such a modest level makes the finding particularly noteworthy from a clinical perspective. Vitamin D is most commonly associated with calcium metabolism and skeletal health. Evidence is growing, however, that it also plays a meaningful role in how the immune system responds to disease. 

Most participants entered the trial with levels below 20 nanograms per milliliter, against a clinical target of 40 to 70 nanograms per milliliter. As the trial progressed, vitamin D levels climbed in the supplemented group throughout the chemotherapy period. The research team viewed those rising concentrations as a potential contributor to the improved outcomes observed. 

Access and affordability give this finding particular practical significance. Some agents used to improve chemotherapy response are expensive and beyond the reach of patients relying on Brazil’s national public health system alone. That gap matters in countries where the public health system covers the bulk of cancer care. 

Carvalho-Pessoa described vitamin D as far more affordable than many drugs used alongside chemotherapy. Some of those alternatives are not available through the public system at all. 

Caution is warranted in interpreting these findings, however, as eighty participants is a modest sample from which to draw firm conclusions. If confirmed in larger studies, the finding would carry significant implications for resource-constrained health systems globally. 

The research team has described the results as promising and called for follow-up studies involving a larger patient group. Those studies would aim to confirm the effect, clarify the biological mechanism, and establish whether outcomes vary across different patient populations.
